We are always accepting new patients. If you are interested in establishing care at our office, please fill out the form below. We will contact you as soon as possible to schedule your first visit.
At your first visit, you will fill out intake paperwork detailing your medical and dental history as well as any concerns or problems you may have. We will complete a comprehensive initial exam, take x-rays (or examine any current ones you may already have), and discuss any of the doctor’s findings, any concerns you may have or treatments that you may be interested in.
